Resources for Our Referring Practices
Instructions for Making Referrals
Our specialists are available to your clients by appointment only and as a referral from your practice. We ask that you follow the instructions below when making a referral to ensure we have the most information as possible in order to best serve you and your clients.
1. Fax a completed Referral Notification Form to us at 413-665-4934. Please do not tell your clients to call us directly as we cannot schedule an appointment without a completed Referral Notification Form. This applies to non-emergency ultrasounds as well.
2. Fax or send us all related information including lab work, cytology and biopsy results, x-rays, etc.
3. After we receive the Referral Notification Form we will contact your client to schedule an appointment.
